- Any unanticipated fire or discharge of a fire extinguisher
- Any call for outside help to contain/control the fire

Animal |
- Animal bite or scratch
- Bodily fluid splash into eye, mouth, nostril, or open wound

Biological |
- Exposure to biological material through a needle stick or cut
- Splash of biological material on mucus membrane (eyes, nose, and mouth)
- Any other event where someone believes he/she has been exposed to a biologically hazardous material

Chemical |
- Any (hazardous) chemical exposure to personnel
- Any spill requiring the use of a spill kit OR requires assistance to be cleaned up

Laser |
- Any eye/skin or other injury from a laser

- Spill of either a high concentration OR high volume radioactive material
